Complete your DOT Physical in 3 Easy Steps
STEP 1: Fill out a brief, one-page health history form.
STEP 2: Undergo your DOT physical exam conducted by Dr. CJ Kosmicki, D.C., a certified examiner for FMCSA. This comprehensive exam includes:
Reviewing medical history and discussing any special medical conditions.
Assessing vital signs, height, weight, blood pressure, pulse rate, range of motion, breathing, eyes, and ears.
Performing a physical exam as outlined in FMCSA guidelines, covering areas such as range of motion, lung function, heart health, abdominal health, orthopedic assessments, and neurological checks.
Conducting a basic urinalysis test (please note this is not a drug test).
Step 3: We handle your paperwork, with a photo ID required. You'll receive:
A wallet-sized, laminated DOT medical card for your CDL, ensuring compliance while driving.
Two copies of the DOT physical form, one for your CDL and one for your company. Upon request, we can also fax the DOT physical form to your company and DMV.
We'll securely store a scanned copy of your DOT physical form at our office for your convenience.
Payment for this comprehensive service is $95.00, and we accept cash, check, or credit card. Please note that health insurance does not cover the DOT physical exam.
